Before you begin, you may consider creating a Google Voice number to protect your information and identity.
This will all happen anonymously to ensure your identity and information are protected. A phone number will be the only thing you must publicly provide as well as zip code to help find donors and allow them to contact you. Your name will remain anonymous except to me.
Please do not list a plant that you are unable to provide verified photos of to avoid being reported as suspicious.

Fill out the following form to register as a Receiver. Your information will not be used for any purpose other than coordinating this effort.

Click the button below and create an account/sign in. You will not be able to interact with the form until you do so.
Press the big red “Click to Start!” button one time— this will create your personal request page. Enter your Zip Code as the new title where it says “ENTER ZIP CODE HERE” at the top of your new form.
Do NOT edit the main template page!
3. list
List all the plants you lost. You must have a verified photo for every plant added to your list. The photo must show the plant and a handwritten note with your phone number to avoid the use of duplicate or fake photos.
Be sure to add this in the “Attach Photo” field for EVERY plant.
4. wait
You’ve done all that you can do! Wait for Givers to reach out to you.
You do not have to accept right away. We understand if you need to delay receiving the plant(s) until the circumstances are appropriate for you.
When you’ve arranged a Re-Leaf successfully, please mark the ‘Re-Leafed’ box so I can keep track. (And send me a photo for the gram!)
